Southern Area Hospice Services applied for funding to the
New Opportunities
Funding (NOF) Body for the development of a Hospice Community Liaison
Sister.
Aim of the Service
The aim of the Community Liaison Sister post is to provide
effective patient centred admissions to Hospice and effective
discharge planning of patients from Hospice to Home, thus
providing seamless continuity of care for the patient.
The Role
The role of the Community Liaison Nurse is to enable the Patient,
Carers, Hospice and Community Services to participate in planning,
to enable a smooth admission and discharge from Hospice. Within
the Hospice the Community Liaison Sister works closely with
all members of the multi-disciplinary team. By liaising with
key personnel the Community Liaison Sister's post provides
effective patient centred admissions to Hospice and effective
discharge of patients from Hospice to Home.
She liaises with the following:
• Patients, their families and Carers
• Hospital Consultants
• General Practitioners (GPs)
• District Nurses
• Macmillian Nurses
• Other Community Health Care Professionals
Offers Advice
The Community Liaison Sister offers advice and support not
only to patients and their families but also to Community
Health Care Staff.

When is the Community Liaison Sister available?
The Community Liaison Sister is available at the Southern
Area Hospice
Monday - Friday, 9am - 5pm.
