The Social Work Service is available to:
• Patients
• Their relatives
• Their carers
who have been referred to the Hospice Unit.
Who can refer to the Social Worker?
• General Practitioners, District Nurses, Macmillan
Nurses and
..Social Workers.
• Patients, their relatives and carers may refer themselves.
• Hospital Multi-disciplinary team members.
How can we help you?
The Social Worker’s main responsibility is to recognise
and respond to the
psychological, social, emotional and practical needs of both
patients and their
relatives and carers.
They are trained to offer support relating to issues of living
with a serious illness,
impending death, loss and grief.
An Independent Social Work Service Providing:
• Liaison with Community Statutory and Voluntary
Services for the provision of
support services to patients and carers at point of discharge.
• Information and advice regarding Social Security
Benefit entitlements, and
offering assistance in completion of application forms.
• Practical resource advice and information.
Family Support / Counselling
Enabling patients, families and carers to:
• Talk and express their feelings about what they
are facing and support one
another.
• Talk about plans and hopes, and assist both to make
important decisions.
• Adjust to new and ever changing circumstances.
