Community Liaison Sister

Hospice to Home

The aim of the Community Liaison Sister post is to provide effective patient centred admissions to Hospice and effective discharge planning of patients from Hospice to home, thus providing seamless continuity of care for the patient.

The Community Liaison Sister will liaise with patients, their families and carers, Hospital Consultants, General Practitioners (GPs), District Nurses, Macmillan Nurses and other community health care professionals.

The Community Liaison Sister is available at the Southern Area Hospice
Monday - Friday, 9am - 5pm.
The Community Liaison Sister will also give information on other Hospice services such as the In-patient Unit, medical homecare visits, outpatients clinic, day hospice unit and bereavement care.
