Liaison Sister

Hospice to Home
The aim of the Community Liaison Sister post is to provide effective patient centred admissions to Hospice and effective discharge planning of patients from Hospice to home, thus providing seamless continuity of care for the patient. This relatively new development for Southern Area Hospice is funded by the New Opportunities Fund (NOF).

Within the Hospice the Community Liaison Sister works closely with all members of the multi-disciplinary team. By liasing with key personnel the Community Liaison Sister's post provides effective patient centred admissions to Hospice and effective discharge of patients from Hospice to home.

The Community Liaison Sister will liase with the following:
• Patients, their families and Carers
• Hospital Consultants
• General Practitioners (GPs)
• District Nurses
• Macmillan Nurses
• Other Community Health Care Professionals

Offers Advice
The Community Liaison Sister offers advice and support not only to patients and their families but also to Community Health Care Staff.

When is the Community Liaison Sister available?
The Community Liaison Sister is available at the Southern Area Hospice
Monday - Friday, 9am - 5pm.

The Community Liaison Sister will also give information on:
• In-patient Unit
• Medical Homecare Visits
• Outpatients Clinic
• Day Hospice Unit
• Bereavement Care
• Physiotherapy
• Patient Helpline
