Social
Work
The Social Worker’s
main responsibility is to recognise and respond to the psychological, social,
emotional and practical needs of both patients and their families. They are
trained to offer support and assess patients and their families' needs relating
to issues of living with a serious illness, impending death, loss and grief.
Who are we?
We are an independent social work service who is employed
by the Hospice. Our team comprises of one full-time senior
social worker and two part-time social workers.
Who is our service available
to?
We offer emotional, psychological and practical support to patients, their
carers and family. We understand that illness can impact on a wide range of
issues for patients and their families.
Our aim is to try to assist you with your present situation and to support you
as you confront issues or challenges that arise.
Who can refer to the
Social Worker?
We accept referrals from General Practitioners, District
Nurses, Macmillan Nurses and Social Workers. Patients,
their relatives and carers can also make a referral for
themselves.
Members of the hospital and hospice multi-disciplinary team can also refer someone
to the social work department.
The ways in which we
help are wide ranging and include:
• Providing a listening ear to patients, their families and carers. Helping
you to explore the thoughts and feelings associated with illness and the changes
it brings.
• Helping and advising patients on talking with their children and grandchildren
about their illness.
• Enabling patients and their families to talk and express their feelings
and to support one another.
• Liasing with Health and Social Care Trusts to secure the provision of
a suitable care package or nursing home placement.
• Referral to other agencies including occupational therapy, Northern
Ireland Housing Executive, Benefits Agencies and assistance with grant applications.
