Community Liaison Sister
We will liaise with you, your family, carers and healthcare professionals including Hospital Consultants, GP’s, District Nurses and Macmillan Nurses to offer advice and support.
The aim of the Community Liaison Sister is to manage patient admissions to Hospice and follow on with effective discharge planning from Hospice to Home, thus providing seamless continuity of care for the patient.
The Community Liaison Sister will liaise with patients, their families and carers, Hospital Consultants, General Practitioners (GPs), District Nurses, Macmillan Nurses and other community health care professionals to offer advice and support. Within Hospice, the Community Liaison Sister works closely with all members of the multi-disciplinary team.
Prior to admission, where possible, the Community Liaison Sister will make a pre-admission home assessment visit and will discuss care options with the patients and/or families. The Community Liaison Sister will also give information on other Hospice services such as the In-patient Unit, Medical Homecare visits, Out-patient Clinics, Day Hospice Unit and Bereavement Support Services.
